Cardiac Disease Management
VNSW has created a Cardiac Disease Management Team that
provides an advanced level of nursing assessment complemented by in-home
arrhythmia and EKG monitoring, oxygen saturation rates, fluid status
measurement and anti-coagulation testing. The EKG weighs 4 oz. and fits
directly into the nurses’ laptop computers, which enables them
to perform an EKG and rhythm strip on the laptop, bedside in the patient’s
home.
In Westchester, a home-based approach to rhythm assessment
is exclusive to VNSW’s Cardiac Disease Management Program. Home visits made
by the VNSW Cardiac Team are covered by insurance as a skilled nursing
visit, with no additional charge to the patient.
