The “VNSW
CARES” Program
VNSW Cares is designed to support and assist older adults and their families in leading a more independent self-directed lifestyle through accessing community resources and long term planning.
When older adults and their families are concerned about:
DIRECTION in planning for long term care, including the possibility of nursing home placement
ALTERNATIVES when nursing home placement is not the answer
TRANSITION when changes in abilities require professional support and expertise
The Program provides for the expertise of a Medical Social Worker, known as the Case Manager, to assist in the assessment and evaluation of an individual’s overall care needs. The Case Manager will discuss care options, support systems available in the community and issues pertinent to the individual’s safety at home. In addition, the Case Manager will explore the options available for assisted living or other suitable placement alternatives. Should an individual and family be interested in nursing home placement, the Case Manager will provide the appropriate information necessary for placement and arrange for the required paperwork (P.R.I. and Screen).

Referrals can be self generated, made by community support personnel, physicians, concerned friends or family members.
