Joint Replacement Therapy

More than a half million Americans will need total knee or hip replacement surgery this year. Prior to surgery, patients may experience a great deal of pain, decreased mobility, difficulty with ambulation and overall loss of functional independence. Following surgery, after a period of rehabilitation, more than 90% of patients return to an independent lifestyle.

The physical therapist provides a comprehensive assessment of the patient and the home environment, pre- and post-surgery. Within 24 hours of a patient returning home post operatively, the physical therapist makes a home visit and performs a comprehensive assessment, confers with the physician and determines the amount of therapy and other services that the patient might need. The occupational therapist will also visit, to determine how patients are doing with their activities of daily living.

Services

  • Comprehensive pre-and post-surgical rehabilitation services to patients undergoing joint replacement
  • Pre-surgical evaluation for home readiness and safety, equipment needs and pre-operative strengthening exercises
  • Physical therapy evaluation and treatment in the patient’s home within 24 hours of hospital discharge (including weekends)
  • Individualized comprehensive treatment plans
  • Patient/caregiver education based on current best practices
  • Frequent communication with the Physician and other disciplines
  • Support system to assist the patient/family with transition to community living
  • Skilled nursing assessment of surgical wound and intervention when indicated; Pain management by nurse specialist; and management of anti-coagulation therapy
  • Home health aide to assist with personal care
  • Occupational therapy evaluation and treatment
  • Assistance by medical social worker to access community resource
