Specialty services assistance with hospital discharge planning, PRI's (Patient Review Instrument and Screen) for nursing home placement, maternal child care services, psychiatric nurse specialists, enterostomal nurse specialists, in-home anti-coagulation monitoring, venipuncture and home intravenous therapy nursing visits, geriatric case management, coordination of medical equipment/supplies and laboratory services, advanced wound care and incontinence management, medication management, enteral (tube) feeding, respiratory therapy, terminally ill care, cardiac care, orthopedic care, diabetic care, AIDS care, wellness programs and more.
