Rehabilitation Services

Therapy where you need it…in your home

With 80-plus rehabilitation professionals, VNS Westchester has the largest team of skilled and licensed home health care therapists in the county. We have helped thousands of patients regain mobility, manage everyday tasks, and speak, swallow and communicate better following surgery, falls, or illness.

Our therapists specialize in specific treatment approaches and have expertise in diagnosing patients who require rehabilitative care to address: post-operative weakness, neurological disorders, cardiovascular diseases, arthritis, joint replacement and other orthopedic impairment, swallowing disorders, and other physical challenges.

Because we are right in the home with you, we can provide an individualized plan of care, adapting our treatment to your environment as well as to your medical needs. To help patients continue to improve between visits, we involve patients, caregivers, family members, and the patient’s physician throughout the rehabilitation process.

We help patients achieve optimal health and independence through the following skilled rehabilitative services:

"RESTORE" - A Program for Joint Replacement Therapy at Home

VNSW provides a comprehensive rehabilitation approach in the treatment of total knee and total hip replacements to restore patients to their highest possible level of physical function. Our program includes post-operative treatment following discharge from a hospital or sub-acute/rehabilitation facility. Our plan of care encompasses the coordinated services of physical and occupational therapists, skilled nursing, pain management, wound care, home health aide and social work services, as indicated. All clinical services are provided under the Physician's orders. Learn more about our RESTORE program.

Physical Therapy

Our physical therapists assess their patients’ level of mobility by evaluating their gait, range of motion, strength and balance. They examine the patient’s physical environment to determine ease of access in moving around their home, and work with patients to help them successfully navigate indoors and, eventually, outside. Safety is of paramount concern, so we make sure patients are equipped with the right personal and home devices to ensure stability, and we teach patients and caregivers how to avoid falls and accidents.

Occupational Therapy

During recovery from surgery or illness, even small tasks can be daunting for patients. Our occupational therapists train individuals to independently perform everyday activities, such as grooming, bathing and homemaking. We help patients to strengthen their fine motor skills and teach them to use adaptive equipment to do as much on their own as possible.

Speech Pathology

Our speech/language pathologists assess a patient’s ability to communicate, which may involve not only speaking clearly, but also recalling names or organizing thoughts. Our expertise in fostering functional communication enables patients to improve their use of words, memory and communication processing. We also work with patients on speech production, articulation, voice problems and facial weakness, as well as swallowing disorders (Dysphagia).

“I’m writing to thank you for the excellent physical therapy and rehabilitation that I received from your therapist, Erika, after my surgery and hospital stay for a fractured femur [thigh bone]. She taught me the skills and gave me the confidence to do my exercises and to walk again on my own, restoring my independence at home.”
